When to use this check

  • You're 18 or over
  • You've felt low, worried, stressed or not sleeping well recently
  • You want to track how you're doing or prepare for a GP visit

  • You are in crisis or thinking of harming yourself — get help above
  • You're under 18 — talk to your GP or a parent/guardian
  • You're already under a mental health team for a severe condition

Part 1 — Mood (PHQ-9)

Over the last 2 weeks, how often have you been bothered by any of the following?

1. Little interest or pleasure in doing things
2. Feeling down, depressed, or hopeless
3. Trouble falling or staying asleep, or sleeping too much
4. Feeling tired or having little energy
5. Poor appetite or overeating
6. Feeling bad about yourself — or that you are a failure or have let yourself or your family down
7. Trouble concentrating on things, such as reading or watching television
8. Moving or speaking so slowly that other people could have noticed — or the opposite, being fidgety or restless
9. Thoughts that you would be better off dead, or of hurting yourself in some way

Part 2 — Anxiety (GAD-7)

Over the last 2 weeks, how often have you been bothered by the following?

10. Feeling nervous, anxious or on edge
11. Not being able to stop or control worrying
12. Worrying too much about different things
13. Trouble relaxing
14. Being so restless that it is hard to sit still
15. Becoming easily annoyed or irritable
16. Feeling afraid as if something awful might happen

When to see a GP

  • A score of 10 or more on either questionnaire
  • Symptoms lasting more than 2 weeks, or getting worse
  • Difficulty working, studying, sleeping or looking after yourself
  • Using alcohol or drugs to cope

Common questions

Is this a diagnosis of depression or anxiety?

No. The PHQ-9 and GAD-7 are screening questionnaires used by GPs to measure how much symptoms are affecting you. Only a clinician can make a diagnosis after talking with you.

Who should use this check?

Adults aged 18 and over who have felt low, anxious, worried or stressed over the last two weeks and want to understand how much it is affecting them.

Who should not rely on this check?

Anyone in crisis or having thoughts of suicide or self-harm, anyone under 18, people with hearing voices or very unusual experiences, and people already under a mental health team for a severe condition. Please seek direct help instead.

Is what I enter stored?

No. Your answers stay on your device and are never sent or saved. We only count anonymously that the tool was used.

Based on the PHQ-9 (Kroenke et al., 2001) and GAD-7 (Spitzer et al., 2006). These are screening tools that measure symptoms over the last 2 weeks; they do not diagnose depression or anxiety. Nothing you enter is stored, sent or logged.
