Asthma is a chronic inflammatory airway condition characterised by variable symptoms such as wheezing, shortness of breath, chest tightness, and coughing. The Global Initiative for Asthma (GINA) guidelines recommend that all adults and adolescents with asthma should receive an inhaled corticosteroid (ICS)-containing controller treatment rather than relying solely on short-acting beta-agonists (SABA) like blue inhalers.


What are the common symptoms of asthma?

Asthma symptoms vary over time and can change in intensity. Common symptoms include wheezing, which is a whistling sound when breathing out. Patients frequently experience shortness of breath and a tight feeling in the chest. A persistent, dry cough is common, particularly at night or early in the morning.

These symptoms are often triggered by exercise, cold air, allergens, or viral respiratory infections. In Ireland, damp weather conditions and high pollen counts can exacerbate these airway symptoms.

What do the GINA guidelines recommend for asthma management?

The Global Initiative for Asthma (GINA) regularly updates its evidence-based strategy for asthma management. A major change in the recent GINA guidelines is the recommendation against treating asthma with short-acting bronchodilators (SABA) alone. Using a blue reliever inhaler alone increases the risk of severe asthma attacks and asthma-related death.

GINA now recommends that all adults and adolescents receive symptom-driven or daily inhaled corticosteroid (ICS) therapy. This approach targets the underlying airway inflammation from the very beginning of treatment. The preferred first-line controller therapy for mild asthma is a combination of low-dose ICS and formoterol, used as needed.

Why is the GINA track approach important?

The GINA guidelines outline two main treatment pathways, known as Track 1 and Track 2. Track 1 uses low-dose ICS-formoterol as both the controller and the reliever therapy, which is the preferred clinical approach. When a patient experiences symptoms, they use their ICS-formoterol inhaler, which provides rapid relief while simultaneously delivering protective anti-inflammatory medication.

Track 2 is an alternative pathway where the patient takes a regular daily controller inhaler and uses a SABA inhaler for symptom relief. Your GP will help determine which track is safest and most effective for your specific asthma severity.

Current GINA guidance advises against treating adults and adolescents with a short-acting blue reliever inhaler alone because it does not treat underlying airway inflammation. Your GP can review your symptoms, reliever use and current preventer treatment to recommend an appropriate plan.
